fix: add migration for schema drift accumulated via db push
...
schema.prisma had fields (Employee.permissions/roles/locations/
onboardingToken, Business.stripeAccountId/slug, CourseBooking
payment fields, GuestLead table, several indexes/FKs) with no
corresponding migration file - they'd only ever been applied via
db push directly against production. Generated via `prisma migrate
diff` against a fresh DB seeded with only the committed migrations,
verified clean apply with `prisma migrate status`.

Adds the staging project's storage hostname to images.remotePatterns
so uploaded avatars/course images resolve on the Vercel preview
deployment. Production hostname stays as-is.

fix: use process.env directly in prisma.config.ts
...
Prisma's env() config helper throws synchronously during config
load if the variable isn't yet visible, which crashes on Vercel
before dotenv/config or the platform's injected env vars take
effect (known Prisma 6.19+/7 issue). process.env access defers
validation to the actual command instead.
