ed363475ca fix: add migration for schema drift accumulated via db push 
schema.prisma had fields (Employee.permissions/roles/locations/
onboardingToken, Business.stripeAccountId/slug, CourseBooking
payment fields, GuestLead table, several indexes/FKs) with no
corresponding migration file - they'd only ever been applied via
db push directly against production. Generated via `prisma migrate
diff` against a fresh DB seeded with only the committed migrations,
verified clean apply with `prisma migrate status`.
